We live in the state, so a family event must be recorded in state bodies, and a little man should get all the documents in the near future. Documents from the hospital

In fact, the baby receives the first documents already upon discharge from the hospital. They confirm the fact of his birth and serve as the basis for the issuance of other documents, and give mom the opportunity to be observed in the antenatal clinic after delivery.

Discharging from the hospital, mom has in her arms:

1. Part of the exchange card. With an exchange card, a woman is taken to the hospital. Part of this card is filled in the hospital and issued to mom:

  • The second sheet, in which there is information contains information about the course of childbirth and the postpartum period. It must be provided to the antenatal clinic, where pregnancy was carried out, within 10 days after discharge.
  • The third sheet, which contains information about the newborn baby: its parameters at birth and at the time of discharge, an assessment of its condition, information about vaccinations given, and the application to the breast. This document is required for a children's clinic. It is hemmed to the child’s card. It is best to pass a sheet with information about the baby to the nurse or doctor who is carrying out the patronage of the newborn.

It is worth noting that both of these documents can be in a different form, not as part of an exchange card. The fact is that the “exchange” does not have a strict form, some cards may not contain the necessary sheets at all, so all the necessary information from the hospital is transmitted on other forms.

2. Birth certificate of a child (certificate of extract, medical birth certificate). Without this certificate, further paperwork is not possible, it is she who confirms the fact of the birth of a child to a particular woman. It records where, when, and by whom the baby was born, what gender it is, which staff took birth.

3. The third coupon of the generic certificate and its annex. The coupon itself remains at home, and two annexes to it (3-1 and 3-2), together with information about the newborn, are transferred to the children's clinic. This is necessary to compensate for the cost of medical observation of the baby in the first year.

Birth certificate

Obtaining a baby’s birth certificate is possible in any registry office: at least at the place of registration, at least at the place of actual residence, if the registration and current place of residence do not coincide. For this, mom or dad needs to come to this institution with the necessary documents within the first month after birth.The registry office must submit the following documents for registration of a newborn:

  1. Birth certificate issued at the hospital.
  2. Passports of both parents (or only one of them).
  3. Marriage document (if the marriage is official).

To receive a birth certificate, you will need to write a statement. Help to do this in the registry office. Data on the date, place of birth, parents are entered in the birth certificate of the child. The surname is assigned by the surname of one of the parents, and the name is given by the parents. Also, at the request of the parents, the nationality of the baby can be indicated there.

A birth certificate is issued quickly: it is usually ready already on the day of treatment. Together with the birth certificate in the registry office issue a birth certificate. This document has a uniform form F-24 and is needed to apply for a one-time allowance for the birth of a child.

Registration and citizenship

A newborn child can be registered only where the parents are registered or one of them, if they are registered at different addresses. This is done in the FMS. To register, you must submit the following list of documents:

  1. A statement by any of the parents about the need to register a newborn at the place of registration of the mother (father).
  2. Extracts from the personal accounts and house books of the EIRC and the passport office from the place of residence of the pope or mother.
  3. A certificate from another parent certifying that the baby is not registered with him (in case the parents are registered in different places).
  4. Baby's birth certificate (original and copy).
  5. Passports of parents (originals and their copies).
  6. Marriage document (if registered).
  7. A statement from the second of the parents, in which he confirms that he is not against registering the child.

For registration, the presence of the parent whose child will be registered is required. Unlike registration of a birth certificate, registration takes longer and requires a lot of trouble. As for the deadlines, there are contradictions in the laws on this issue: as a newcomer to the place of registration, the child must be registered within 7 days from the date of birth, however, without a birth certificate, registration at the place of residence is impossible, and registration of a birth certificate is not possible given a month. In addition, there is another norm of the law: if a person plans to stay in the area for less than 90 days, he does not need to register, so conditionally the first 90 days a child can live without registration.

Citizenship is also made in the department of the Federal Migration Service, so citizenship and registration are usually carried out simultaneously.

The registration itself is done for several days. For this period, the passports of the parents are in the FMS. In the hands of parents receive a certificate of registration at the place of residence.

Policy of compulsory medical insurance

This paper is needed so that the child can receive the free medical care that was put to him. The right to compulsory health insurance is absolutely all citizens of the Russian Federation. It is also possible to independently choose an insurance company. The policy of compulsory medical insurance is drawn up at the actual place of residence, it is not tied to registration at all.

In order to order an MHI policy for a newborn, you must contact the office of the insurance company (usually they are available directly on the territory of the clinic) and submit the documents:

  1. Identity card of the child, that is, his birth certificate.
  2. Passport of one of the parents (the one who submits the documents).

Mom or dad will need to fill out an application form for an MHI policy. A permanent policy is prepared for about a month, however, after receiving documents for a policy, a temporary certificate is issued.

On this, obtaining documents for the little man ends. It is not worth forgetting or ignoring the execution of any documents, because administrative fines are threatened for violation of the law. 1. Birth certificate

Contact the registry office. Documents:

4. Compulsory health insurance policy

Contact the clinic. Documents:

  • the passport;
  • birth certificate.

(Note that, by law, a child has the right to receive free medical care at any district clinic, regardless of registration.)

5. One-time allowance at birth (12,405 rubles) + allowance for registration in early pregnancy (465.20 rubles)

Contact your work, place of study, or social security if you aren’t working. Documents:

  • certificate from the spouse's place of work that he does not receive this allowance; if the spouse officially does not work anywhere - a copy of the work book and a certificate from the social security that he did not receive benefits there;

6. Monthly allowance for childcare up to 1.5 years

A monthly allowance is assigned to someone who sits directly with the child. It is paid in the amount of 40% of the average monthly salary. Contact your parent's workplace or place of study. Documents:

  • an application for the provision of parental leave and the assignment of benefits and compensation;
  • birth certificate;
  • certificate from the spouse's place of work that he does not receive this allowance;

  • copy of the order on dismissal or liquidation of the organization;
  • employment history;
